Case Manager / Macomb Case Management/ Full-time (40hrs.)/ Days/ Mount Clemens MI

Coordinate all services to maximize consumer recovery and independence for individuals with mental illness by performing home visits and working in the community. Ensure consumer involvement in the Person-Centered Planning Process and facilitate the implementation of Individual Plans of Service. Conduct comprehensive psychosocial assessments, communicate with service providers within Treatment Plans, and coordinate intakes, discharges, and transfers. Facilitate hospitalizations, attend hospital meetings, and participate in legal proceedings including petitioning and testifying in clinical emergencies. Monitor activities such as employment, education, volunteerism, and family caretaking while providing referrals to appropriate community resources. This role supports the Hope Network's mission to build independence through direct, empathetic engagement with consumers and their support systems.
