Job Summary

Conduct client-centered, strength-based rehabilitation and recovery services for adults and transition-age youth with mental illness, including hands-on assistance with daily living skills like hygiene, meal preparation, and travel. Develop and maintain individualized treatment plans, monitor medication compliance, and provide supportive counseling in 1:1 or group formats to modify behaviors and attain personal goals. Perform crisis assessment and intervention, accompany consumers to medical and social appointments, and advocate for resources while ensuring regulatory compliance and safety protocols. Document all case notes, incident reports, and service progress accurately. May cover duties of a residential counselor or another case manager during short staffing and function as a floor supervisor when assigned.

Required Qualifications

  • Bachelor's degree in a human services field
  • Associate's Degree in a human services field and two years of full-time experience providing direct services to individuals with mental disabilities or other relevant human service experience
  • High school diploma or equivalency diploma recognized by the NYS Department of Education, and four years of full-time human services work experience
  • An equivalent combination of training and experience
  • 30 college credits may be substituted for 1 year of human services experience
  • Degree awarded by a regionally accredited college or university or one recognized by the NYS Education Department as following acceptable educational practices
  • Independent verification of equivalency for degrees awarded by educational institutions outside the United States and its territories
  • Ability to work with consumers/residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences
  • Ability to serve as a role model to residents/consumers
  • Ability to develop, evaluate, implement and modify a case management plan, meeting all deadlines and productivity standards
  • Ability to complete written forms and reports in an accurate and timely manner
  • Ability to communicate effectively with staff, consumers/residents, families, and the public
  • Ability to prepare accurate and timely documentation, reports and other written material as assigned
  • Ability to secure the cooperation of and work effectively with others
  • Ability to work independently, and to conform to all applicable safety and accountability measures

Desired Qualifications

  • Basic knowledge of mental illness and serious emotional disturbances and substance abuse disorders
  • Basic knowledge of treatment, rehabilitation, and community support programs as they relate to consumers/residents, families, and staff
  • Basic knowledge of routine clinical procedures and medications
  • Basic knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques
