Job Summary
Manage admission to discharge for all patients, ensuring quality and resource management across the continuum of care. Conduct holistic reviews of medical records to verify medical necessity, service intensity, and severity of illness while promoting cost containment. Integrate healthcare providers to achieve seamless delivery and clinical outcomes. Requires BSW degree or LVN license with acute care experience. Full-time day shift.
Required Qualifications
- BSW or other bachelor's degree in a clinical healthcare related field
- At least one year experience in case management, discharge planning or nursing management
- LVN/LPN graduate from an accredited school of nursing, with current LVN/LPN license, and 2 years of acute care experience
- Experience and knowledge in basic to intermediate computer skills
Desired Qualifications
- Minimum 5 years of acute care experience
- Case management certification within 2 years of hiring
- At least one-year experience in case management, discharge planning or management
- Current BCLS certificate
- Knowledge of Milliman Criteria and InterQual Criteria
