Job Summary

Case Manager responsible for coordinating services for individuals with mental disabilities, traveling to consumers’ residences to provide supportive services, counseling, and development of individualized service plans. Works with an interdisciplinary team to assess needs (medical, psychosocial, educational, financial, vocational) and to advocate for necessary services. Maintains case records, documents progress, reviews service plans, and ensures culturally competent care. Involves direct client interaction, home visits, and collaboration with external care coordinators and ICL staff; requires ability to complete documentation, conduct cognitive-behavioral techniques, and facilitate 1:1 or group counseling while adhering to regulatory guidelines. Must possess a Bachelor’s degree and NYS driver’s license. Location: Brooklyn, NY. On-site role with regular visits to consumer residences; full-time schedule.

Required Qualifications

  • Bachelor’s degree plus 2 years’ experience providing direct services to individuals with mental disabilities
  • Current valid NYS driver’s license
