Job Summary
Coordinate patient care plans by meeting with administration, medical, and nursing staff to develop services for patients and families. Partner with care teams, social agencies, and community resources to ensure smooth discharge transitions. Assist with admission and discharge processes while ensuring all documentation is informative and follows Center policies. Attend care plan meetings to provide professional guidance on patient needs. Maintain highly confidential information in strict compliance with HIPAA regulations.
Required Qualifications
- A minimum one year case management experience
- Leadership ability
- Willingness to work harmoniously with other personnel
- Good judgement
- Ability to maintain highly confidential information
- Knowledge of HIPAA compliance and requirements
- Ability to plan, organize, develop, implement and interpret the programs, goals and objectives of the department as it relates to the Company
- Ability to pass a pre-employment background check/drug screen
- Active Fingerprint card
