Job Summary
Coordinate long-term care across home and nursing facility settings for DSHP Plus LTSS and DSNP members; conduct regular in-home and nursing facility visits, assess and develop individualized care plans, coordinate HCBS benefits and Medicaid, educate members and caregivers about health options, collaborate with interdisciplinary teams to ensure safe transitions between home, community, and facility-based care, authorize LTSS services based on assessments, document services in the electronic health record, and travel within the assigned Delaware territory to member homes and facilities. The role requires proficiency in care coordination, familiarity with LTSS/HCBS/Medicaid, ability to work with geriatric and potentially behavioral health populations, adherence to HIPAA and privacy policies, and the ability to meet regulatory deadlines. The position is full-time, telecommuter-friendly with 25-50% travel, and emphasizes delivering care in members’ homes and nursing facilities while maintaining compliant documentation and care plans.
Required Qualifications
- Bachelor's degree in Social Work or in health, human, or education services OR Master’s degree in Social Work or in health, human, or education services
- 3 years of experience in long-term care, home health, hospice, public health, or assisted living OR Current State RN or LPN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) and 2 years of experience in long-term care, home health, hospice, public health, or assisted living OR A high school degree or equivalent and three years of qualifying experience with case management of the aged, including management of behavioral health conditions, or persons with physical or developmental disabilities, or HIV/AIDS population.
