Job Summary
Case Manager, LINK provides outreach and case management to people experiencing homelessness, assisting with assessments, benefits, psychosocial evaluations, and transition to permanent housing. Responsibilities include connecting clients to psychiatric and medical/substance use treatment, obtaining identification and income sources, developing weekly service plans, preparing housing applications, maintaining records, and crisis intervention. Requires direct client services with a caseload, harm reduction experience, and strong collaboration with social services, with a 35-hour work week.
Required Qualifications
- Bachelor’s Degree or equivalent work experience
- Two years of experience working with homeless adults
- Valid NY, NJ, CT, or PA driver’s license preferred
- Excellent computer skills, including proficiency in Microsoft Word and Excel and the ability to learn new programs
