Job Summary
Case Manager position in mental health focused on identifying client needs by assessing their situation and available resources; coordinates planning services, assists with applications, links clients to service systems, educates clients on program requirements, options, and determinations; monitors service delivery through follow-up interviews; provides emergency/crisis intervention by referrals to appropriate programs; maintains client records and completes required paperwork; ensures confidentiality and compliance with federal, state, and local requirements; fosters inter- and intradepartmental cooperation while supporting transportation arrangements and ongoing client support.
Required Qualifications
- High school diploma or equivalent with counseling experience and/or college course work in psychology or related field
