Job Summary
The Case Manager - Life Skills Facilitator will assist in providing quality care for individuals with mental health and substance use disorders. Responsibilities include developing client-centered care plans, linking clients to community resources, facilitating weekly Life Skill groups, monitoring client behaviors, and conducting 1:1 sessions. The position also requires maintaining professional boundaries, adhering to documentation standards, and participating in staff meetings. The role necessitates a Bachelor’s degree in behavioral science and two years' experience in case management with excellent communication skills and the ability to work independently.
Required Qualifications
- Bachelor’s degree in behavioral science
- Two years’ experience in case management preferred
- Valid Connecticut Driver’s license
