Job Summary
RN Case Manager Liaison Nurse for inpatient discharge planning, onsite at St. Peter Hospital in Olympia; night shift 10 PM–10:30 AM with alternating weekend rotations and holidays; sign-on bonus for eligible external hires; focuses on discharge planning, follow-up care, and care coordination across KFHPW and contracted providers; collaborates with primary and specialty care teams to develop patient-centered discharge plans, ensure adherence to medical plans, and optimize resource utilization; requires ER or case management experience and appropriate RN licensure for Washington; responsibilities include data collection, ongoing assessment, care coordination, and communication with patients, families, and care teams; includes opportunities for involvement in home health care referrals, care conferences, and complex care planning.
Required Qualifications
- Minimum three (3) years of recent RN medical/surgical/ambulatory clinical experience
- Minimum two (2) years of RN experience in ambulatory case management, care coordination or disease management
- Bachelor's degree
- Registered Nurse License (Washington) required at hire OR Compact License: Registered Nurse required at hire
- Basic Life Support required at hire
- Case Manager Certificate within 36 months of hire
