Job Summary
RN Case Manager Liaison Nurse for inpatient discharge planning, onsite at St. Peter Hospital in Olympia, with a 30 hrs/wk schedule (Variable Mon-Fri, 10PM-10:30AM, every other weekend, alternating holidays). Primary duties include assessing patients for case management, developing patient-centered care plans, coordinating with PCP and care teams, facilitating referrals to home health, long-term care, hospice, and other services, and evaluating progress to meet goals. Requires Bachelor's degree, RN license (Washington or Compact), Basic Life Support, Case Manager Certificate within 36 months of hire, and a minimum of three years RN clinical experience plus two years in ambulatory case management/care coordination/disease management. Preferred: two+ years in utilization review or ambulatory case management; BSN.
Required Qualifications
- Bachelor's degree
- Registered Nurse License (Washington) or Compact License
- Basic Life Support
- Case Manager Certificate within 36 months of hire
- Minimum three (3) years of RN medical/surgical/ambulatory clinical experience
- Minimum two (2) years of RN experience in ambulatory case management, care coordination or disease management
