Job Summary
Conduct direct case management for approximately 25 active clients at the Multi-Service Center, including individual peer counseling, advocacy, and accompaniment. Evaluate client needs to create prioritized service plans and coordinate care with service providers to meet goals. Perform new client screenings by reviewing applications and interviews, then enter data into the ONE database within 48 hours. Observe clients using Motivational Interviewing to identify intervention needs and facilitate support groups or meetings as required. Maintain accurate electronic records in Salesforce and daily logs while participating in staff meetings and on-going training. Problem solve with clients to determine assistance strategies and develop exit surveys for program analysis.
Required Qualifications
- BA degree in social services, psychology, or another related field
- Equivalent work experience in lieu of a degree
- Case management experience
- Two years' experience providing services to chronically homeless
- Demonstrated skills in counseling, case management, and client advocacy
- Familiarity with the philosophy and approach of Harm Reduction, Motivational Interviewing and OARS
- Strong interpersonal, verbal, and written communication skills in English
- Understanding of homeless issues, especially in context to race, class, gender, and sexual identity
- Understanding of Housing First Approach to care for homeless population
- Understanding of coordinated entry system
- Knowledge, experience, and comfort working with a diverse community, including LGBTQQ, youth, immigrant, low-income communities, and communities of color
- Proficient in using MS Office including Word, Outlook, and Excel
- Working knowledge of community resources and familiarity with SF homeless shelter systems and services
- Demonstrated commitment to open, direct, and non-violent communication
- Ability to de-escalate crisis situations and make decisions under pressure
- Proficient reporting skills including data entry, Excel, and Word processing
- Strong initiative and ability to work both independently and cooperatively with staff and management
- Ability to maintain professional boundaries with clients, volunteers, and staff
- Commitment to the mission of St. Vincent de Paul Society of San Francisco
- Covid vaccination
- Negative result on TB test
- Background Check completed upon acceptance of job offer
- Monday through Friday availability
