Job Summary
Coordinate and manage individualized care plans while connecting residents with medical, behavioral health, educational, vocational, and community resources. Provide counseling, life skills training, and recovery-focused support, then monitor resident progress and maintain accurate documentation. Collaborate with families, providers, and interdisciplinary teams, and transport residents to appointments and community activities as needed. Starting pay is $23.00+/hour with 401(k) matching, medical, dental, and vision insurance, plus paid time off.
Required Qualifications
- Bachelor's degree in Social Work or a related field, or at least 2 years of mental health experience
- Valid driver's license (if driving is required)
- Ability to pass DOJ/FBI background clearance, TB screening, and health clearance
Desired Qualifications
- Knowledge of Title 22, HIPAA, and Community Care Licensing regulations preferred
