Job Summary
Case Manager role focusing on coordinating services for clients under an Individualized Treatment Plan (ITP). Responsibilities include applying basic clinical interventions to support sobriety and mental health, maintaining up-to-date community-resource referrals, submitting progress notes within 24–48 hours, achieving productivity goals, keeping audit-ready case files, collaborating with referral sources and legal entities, and utilizing crisis de-escalation techniques. Must uphold agency mission and ethics, and perform other duties as assigned. Requires High School diploma or equivalent, 3+ years in social work, preferred chemical dependency certification or licensure, First Aid/CPR/de-escalation training within 90 days, age 23+, valid driver’s license with acceptable driving record, Ohio or Michigan driver’s license, Excel proficiency, and ability to create reports.
Required Qualifications
- High School Diploma or equivalent
- 3+ years of experience in social work
- Chemical Dependency certification or Social Work licensure preferred
- First Aid, CPR, and De-escalation Training certified within 90 days of starting
- Be at least 23 years old, valid driver’s license, acceptable driving record
- Current Ohio or Michigan driver’s license, good driving record, insurance, willing to use personal car
- Computer skills including Excel and ability to create reports
