Job Summary
Conduct comprehensive assessments and develop individualized service plans for residents with chronic medical conditions, mental illness, and substance use challenges. Provide integrated counseling, facilitate group programs, and actively engage clients through apartment visits and accompaniment to appointments. Collaborate with interdisciplinary teams to link clients to medical, mental health, and employment providers while maintaining accurate, confidential documentation. Develop safety plans, de-escalate crises, and work with Leasing and Compliance teams to ensure housing stability. Requires a Bachelor's degree with strong clinical knowledge, bilingual Spanish/English preferred, and experience managing diverse caseloads in supportive housing environments.
Required Qualifications
- Bachelor's degree (BA/BS) from an accredited institution
- Minimum of one year of experience working with adults and/or families impacted by poverty, chronic homelessness, mental illness, substance use, and/or medical issues
- Ability to work independently as well as collaboratively as part of an interdisciplinary team
- Strong organizational skills with the ability to manage a diverse caseload of clients with diverse needs
- Ability to work effectively under pressure, including in crisis situations, and demonstrate flexibility with work schedules when necessary
- Experience using Case Management electronic record-keeping systems
Desired Qualifications
- Master of Social Work (MSW)
- Bilingual Spanish/English
- Strong clinical knowledge and experience working with individuals and/or families impacted by mental illness, substance use, medical issues, and involvement with legal or criminal justice systems
