Job Summary
Provide case management and psychiatric rehabilitation services to clients in a residential treatment setting; assess client needs, make referrals to community resources, and link clients to appropriate services. Monitor client condition and progress, advocate for client access to resources, and coordinate with therapists and treatment teams. Conduct readiness assessments and skills training (individual and group), support clinicians in group activities, and aid in developing comprehensive treatment plans. Maintain liaison with state/local hospitals and community partners, contribute to program development for supported employment, document treatment activities on time, participate in supervision and planning meetings, and uphold professional standards and ethical behavior in interactions with clients and colleagues. Strong computer skills and attention to detail, with ability to manage multiple tasks and maintain confidentiality in a residential facility environment.
Required Qualifications
- Bachelor's Degree in Psychology, Social Work, or related field
- Two or more years of Case Management experience
