Job Summary

Conduct comprehensive in-home and in-office assessments of clients' social and emotional needs using standardized tools. Develop and execute written case plans while providing individual and family counseling to preserve client autonomy. Screen clients for entitlement eligibility, complete applications, and advocate to secure Medicare, Medicaid, and public assistance benefits. Arrange critical direct services including homecare, transportation, meals on wheels, legal aid, and emergency psychiatric or medical care. Evaluate clients' capacity to manage affairs, protect seniors from financial exploitation, and assist with guardianship or emergency relief fund disbursements. Organize daily community activities, facilitate group counseling, and supervise community aides, Title V workers, homemakers, interns, and volunteers. Maintain precise electronic case records, reports, and statistics while representing JASA's mission at community conferences.

Required Qualifications

  • BSW, BA, or BS degree from an accredited college or university
  • Proficiency in English
  • Ability to travel to clients' homes using mass transit
  • Ability to navigate physical barriers (stairs, etc.) in various residences

Desired Qualifications

  • Knowledge of Yiddish, Russian, Spanish, or Patois
  • A deep understanding and appreciation of how race, religion, ethnicity, sexual orientation, and individual values shape service delivery for diverse families
