Job Summary
Case Manager participates as a member of the Jail-based program to provide intensive case management support to patients by initiating services while incarcerated and to transition services into the community. Works within a multidisciplinary team inside detention facilities to help individuals with mental health or substance use disorders access needed medical, social, educational, and other services, and to provide care coordination and care transition services. Requires a bachelor’s degree in a recognized mental health field and relevant experience; salary starts at $21/hour with locale differential and increases with experience and education. Full-time position with comprehensive benefits and travel between Axis Health System locations.
Required Qualifications
- Bachelor’s degree in one of the recognized mental health professions
- one-year or more human service-related experience preferred
- knowledge of patient health information and HIPAA compliance
- excellent interpersonal and communication skills
- ability to be self-directed
- ability to follow confidentiality and security rules
- proficiency with MS Word, MS Excel, MS Outlook, and internet access
- ability to work independently in fast-paced settings
- current driver’s license and insurability for commercial vehicle insurance
- occasional travel
- BLS certification within 90 days of hire
- annual flu immunization and TB screening within 90 days of hire
- de-escalation training within 90 days of hire
