Job Summary
Provide intensive case management to highest-acuity clients experiencing homelessness, including those with serious mental illness and co-occurring needs. Conduct comprehensive, strengths-based assessments and develop individualized service plans covering housing, benefits, healthcare, and long-term stability. Coordinate care across behavioral health, medical, housing, and legal systems while identifying gaps to advocate with internal leadership and community partners. Work closely with clinical providers for discharge planning and post-hospital follow-up, offering advanced non-clinical crisis response and safety planning. Serve as a subject-matter resource to junior staff navigating complex cases. Maintain accurate documentation and ensure compliance with agency and funder requirements. This senior-level role supports continuity of care through crisis and hospitalization, working within Catholic Charities of Acadiana to help clients achieve stability and self-sufficiency.
Required Qualifications
- Bachelor's Degree in Social Work, Psychology, Human Services, or related field
- Minimum 4 years experience in case management, homeless services, or behavioral health systems
- Direct experience with highest-acuity and co-occurring populations
- Deep working knowledge of behavioral health conditions, treatment modalities, and levels of care
- Demonstrated ability to coordinate clinical services
- Demonstrated experience collaborating with clinical and behavioral health providers as part of an interdisciplinary care team
- Strong knowledge of housing systems, benefits systems, and community resources
- Ability to identify and clearly articulate systems-level service gaps to partners
- Excellent interpersonal, communication, and advocacy skills
- Compassionate, nonjudgmental approach
- Proficiency with electronic case management systems and standard office software
- Ability to manage high-acuity, unpredictable situations with professionalism and calm
- Commitment to continuous learning, professional growth, and ethical excellence
- Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects
