Case Manager Intensive (1529)

Job Summary

Conduct weekly in-person trauma support for clients, focusing on crisis intervention, safety planning, and family preservation. Deliver client-centered intensive case management for vulnerable individuals with medical, psychological, or family crisis needs. Perform comprehensive intakes for eligible participants and develop goal-oriented self-sufficiency plans addressing long- and short-term needs. Maintain regular communication with supervisors regarding client safety, stability, or well-being, while submitting detailed documentation in compliance with LSF policies. Screen for financial assistance eligibility, facilitate disbursement, and manage client funds and check requests. Collaborate with other case managers to coordinate services for dual-enrolled participants and guide volunteers through referrals and mentorship. Provide indirect supervision to Program Interns and volunteers, participate in stakeholder meetings, and prepare required program reports. Maintain comprehensive case files ensuring confidentiality and assist with RFPs and project goals.

Required Qualifications

  • Bachelor's degree
  • Must possess a clean/valid Florida Driver License
  • reliable transportation
  • proof of insurance
  • Two years of experience providing case management and/or social services
  • Must demonstrate sensitivity to our service population's cultural and socioeconomic characteristics and needs
  • Must be a team player with monitors as well as other clerical staff
  • Accurate, complete and timely submission of required reports and documentation to LIRS & LSF
  • Effective working relationship with clients (providers)
  • Adherence to LSF policies and procedures
  • Must be able to keep confidential information

Desired Qualifications

  • Preference for degree in a Social Work or behavioral/human services field
  • Two years of experience providing case management and/or social services preferred
  • Case management experience is strongly preferred
  • 3-year experience working with refugee or immigrants preferred
  • Cross-cultural or international experience preferred
  • Bilingual preferred
  • Proficiency in Microsoft Office applications
  • Knowledge of community resources and ability to connect families to community services
  • Excellent verbal and written communication skills
  • Critical assessment and analysis skills
  • Ability to work and deal a diverse group of clients
