Job Summary
Case Manager/Intake Coordinator responsible for coordinating, planning, and facilitating health and wellness programs for first responders; serves as a bridge between clinical services, wellness services, and community resources; conducts initial intake screenings and wellness assessments to determine client eligibility, manages referrals and a comprehensive referral network, and develops recovery plans with psycho-social-emotional supports. Requires strong documentation in Salesforce, ability to coordinate with a multidisciplinary team, provide crisis intervention when needed, and promote community integration and independent living. Emphasis on data management, reporting, and administrative duties to support program operations.
Required Qualifications
- Undergraduate degree in business administration, Non-Profit management, public health or related field from an accredited college or university, or relevant work experience can be used in place of degree
- 3 years of experience relevant to the position and working with vulnerable populations
- Driver’s License must be valid and must be able to successfully pass a criminal background check
