Case Manager, Inpatient

Coordinate patient discharge plans with interdisciplinary teams, assessing needs and aligning transitions with patient/family goals of care. Maintain effective inter-professional collaboration across clinical teams to ensure safe transitions to the next level of care. Collaborate with social workers on complex psychosocial needs, report quality-of-care and risk management issues to the Director, and identify community resources for financial, transportation, and pharmaceutical assistance. Identify and report quality-of-care and risk management issues to the Director in a timely fashion. Maintain familiarity with all hospital and department specific policies and procedures.
