Job Summary
Conduct thorough client assessments and develop individualized service plans addressing housing, employment, education, recovery, legal, financial, medical, and mental health needs. Coordinate services with the VA, landlords, employers, and treatment providers while facilitating life skills and therapeutic groups tailored to specialized veteran populations. Provide regular one-on-one meetings and conduct home visits for project-based and scattered-site housing, assisting with crisis intervention and after-hours support as needed. Lead day-to-day coordination of client services, guide Case Manager I & II team members, and supervise interns. Document all services and critical incident reports in MCR and other electronic systems, conduct drug and alcohol screenings, and support reporting, audits, and file reviews. This advanced-level position helps drive life-changing outcomes for veterans overcoming housing instability, trauma, and mental health challenges while providing leadership to newer staff.
Required Qualifications
- Master's degree in social work, psychology, counseling, or related field
- 2+ years of experience in case management or social services
- Ability to pass background checks if assigned to programs involving incarcerated or recently released clients
- Strong communication, documentation, and organizational skills
- Proficiency in Microsoft Office and electronic records systems
- Valid driver's license
- Ability to conduct home visits and transport clients as needed
Desired Qualifications
- Experience with homeless, veteran, or justice-involved populations
- Certification in Substance Abuse Counseling (e.g., CCAPP, CAADE)
- Active registration or license with the BBS (ACSW, AMFT, LCSW, LMFT)
- Experience facilitating groups or life skills classes
