Job Summary
Case Manager III at Heartland Family Service leads direct care and case management in a residential substance abuse treatment facility serving women and their children. The role provides supervision and support to residents as part of 24-hour direct care staff and delivers case management for assigned families, contributing to education and support programming for women, children, and families. Must have strong trauma-informed approach, collaborate with multi-disciplinary teams, and engage with community resources; candidates with lived recovery experience are preferred (minimum 2 years sustained recovery). Responsibilities include supervising participants, implementing case management plans, intake completion, documenting progress, coordinating with Lead Case Manager and Program Coordinator, maintaining records, ensuring safety, facilitating support groups, and building connections to community services. Requires ability to work independently, be dependable and punctual, and participate in ongoing training; essential functions performed on site at the Omaha, NE location.
Required Qualifications
- Bachelor’s Degree in Human Services, Psychology, Social Work, Sociology or a related field preferred
- Two or more years’ experience in human services field
- Valid driver’s license/ acceptable driving record
- Bilingual-Spanish speaking abilities preferred
