Job Summary
Assist individuals with mental illness and co-occurring disorders to live independently in their chosen community under direct supervision. Provide primary support and case management to help clients navigate appointments, groups, and services using personal or company vehicles. Manage daily interactions for adults, older adults, and those with co-occurring disorders across programs including Westside Stanislaus Homeless Outreach, Partnership TRAC, Josie's TRAC, and MRS TRAC. Work full-time days shifts with varying hours, earning a starting wage of $22.17 to $27.08 based on geographic location.
Required Qualifications
- Bachelor's degree in Social Services or Human Services
- High School Graduation or G.E.D. equivalent
- four (4) years of direct services in Mental Health or community services
- Associate's degree
- Valid and current driver's license
- personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc.
