Job Summary

Conduct assessments to accept, deny, and refer applicants for scattered site Permanent Supportive Housing in Douglas, Sarpy, and Pottawattamie Counties. Verify eligibility, document comprehensive service records, and assist clients in locating and securing housing using a Housing First model. Develop housing stability plans, coach clients on self-advocacy, and provide crisis response services to mitigate trauma histories. Access employment and benefits to increase financial security while ensuring safety during in-home visits in non-traditional settings. Submit timely reports per federal guidelines and collaborate with landlords, referral sources, and community providers. Maintain independence with minimal supervision while adhering to trauma-informed and culturally competent principles.

Required Qualifications

  • Bachelor's Degree in Social Work or Human Service field
  • Valid Driver's License
  • Acceptable driving record
  • Two years direct care experience in Human Services
  • High level of social work skills
  • Practice and knowledge of social work

Desired Qualifications

  • Equivalent experience or education to the Bachelor's Degree requirement
  • Two years direct care experience in Human Services
