Job Summary
Conduct comprehensive in-home assessments and reassessments of older adults' functional capacities, unmet needs, and eligibility for benefits. Develop and implement care plans with seniors and families to address challenges and ensure safe home living. Provide crisis intervention for medical emergencies, utility shut-offs, and disaster response, plus emergency coverage for Home Delivered Meal clients. Perform intake activities including eligibility assessments, preliminary evaluations, and referrals. Complete the DFTA assessment form for each client per funding regulations and maintain up-to-date case notes and monthly reports. Requires a Bachelor's degree in human services, proficiency in Microsoft Office, and ability to travel; offers hourly pay between $32.96 and $36.92.
Required Qualifications
- Bachelor's Degree in Human related services from an accredited college or university
- Able to travel to client's homes, meetings, conferences, and other locations for training
- Ability to lift to 10lbs
Desired Qualifications
- Knowledge of entitlement for older adults preferred
- Proficient in computer software applications, Excel, and Microsoft Office
- Bilingual preferred
