Job Summary
Conduct comprehensive in-home assessments and reassessments of older persons' functional capacities, unmet needs, and eligibility for benefits. Develop and implement care plans with seniors and families to address unmet needs and ensure safe living conditions. Provide crisis intervention for medical emergencies, utility shut-offs, and disaster response, plus emergency coverage for Home Delivered Meal clients. Perform intake activities including eligibility assessments, preliminary evaluations, and referrals. Complete DFTA assessment forms and maintain case notes, waiting lists, and monthly reports per funding regulations. Requires a Bachelor's degree in human services, proficiency in Microsoft Office, and ability to travel and lift 10lbs. Bilingual candidates preferred.
Required Qualifications
- Bachelor's Degree in Human related services from an accredited college or university
- Able to travel to client's homes, meetings, conferences, and other locations for training
- Ability to lift to 10lbs
Desired Qualifications
- Knowledge of entitlement for older adults preferred
- Proficient in computer software applications, Excel, and Microsoft Office
- Bilingual preferred
