Job Summary
Conduct comprehensive in-home assessments of older adults to evaluate functional capacities, cognitive limitations, and eligibility for benefits. Develop and implement care plans with seniors and families to address unmet needs, while providing crisis intervention for medical emergencies, utility shut-offs, and disaster responses. Manage intake activities including preliminary evaluations, service initiation, and referrals. Complete DFTA assessment forms per funding regulations and maintain accurate case notes, waiting lists, and monthly reports. Provide emergency coverage for Home Delivered Meal clients when access is unavailable.
Required Qualifications
- Bachelor's Degree in Human related services from an accredited college or university
- Able to travel to client's homes, meetings, conferences, and other locations for training
- Ability to lift to 10lbs
Desired Qualifications
- Knowledge of entitlement for older adults preferred
- Proficient in computer software applications, Excel, and Microsoft Office
- Bilingual preferred
