Job Summary
Case Manager III at U.S.VETS provides guidance to veterans to help achieve independence. Responsible for assessing barriers across Housing, Employment, Education/Training, Financial, Sobriety Support/Addiction Treatment, Legal, Family/Social, and Medical/Psychiatric areas; coordinating care with VA personnel and community partners; facilitating intake and maintaining client records; developing Individual Action Plans (IAPs) and Discharge summaries; conducting home visits; supervising interns; delivering life-skills groups; and performing data entry and reporting across HMIS/HOMES systems. Requires a master's degree in a related field, experience with homeless and/or veteran populations, strong communication skills, valid driver's license, and willingness to work across multiple HUD/VASH duties.
Required Qualifications
- Masters Degree in Social Work, Psychology, Counseling, or other related social service field required.
- Two years of experience in social services or related field preferred.
- Experience working with homeless and/or veterans preferred.
- Certification in Substance Abuse Counseling preferred.
- Experience facilitating support groups preferred.
- Strong oral and written communication skills.
- Professional manner and appearance.
- Ability to work independently and within a team.
- Ability to take direction.
- Ability to work effectively with diverse group of clients, staff, and community members.
- Computer proficient in Microsoft Office and Internet.
- Valid drivers license required. Must meet company insurance requirements and complete a provided driver training course.
