Job Summary
Assess veterans referred to the program regarding housing, employment, education, financial, sobriety, legal, family, and medical needs. Coordinate care with VA personnel and community partners for eligibility documentation and health services. Facilitate client intake, compile files, and collaborate on Individual Action Plans. Conduct crisis intervention, home visits, and life skills classes while maintaining confidential client files in compliance with organization standards. Provide guidance to junior staff on intervention strategies and progress notes during weekly meetings. Assist with quality management, data collection, program reporting, and training new staff. Support urgent client issues outside business hours and complete timely discharge summaries. Drive company vehicles for client transportation if a valid license and insurance requirements are met.
Required Qualifications
- Master's Degree in Social Work, Psychology, Counseling, or other related social service field
- Valid driver's license
- Intermediate computer literacy
- Ability to pass background for clearance into jails/prisons
Desired Qualifications
- Two years of experience in social services or related field
- Experience working with homeless and/or veterans
- Certification in Substance Abuse Counseling
- Experience facilitating support groups
- Professional manner and appearance
- Ability to work independently and within a team
- Ability to take direction
- Ability to work effectively with diverse group of clients, staff, and community members
