Job Summary
Conduct direct case management for adults with serious mental illness and co-occurring conditions experiencing homelessness or housing instability. Prepare, evaluate, and maintain individualized service plans using Catholic Charities and community resources. Support clients in obtaining services, benefits, and supports while coordinating referrals with community providers. Provide education, resource information, and crisis management services, including in-home visits when appropriate. Collaborate with supervisors and team members for consultation and case planning. Transport clients to appointments or community resources when necessary. Maintain accurate case documentation meeting all agency, funder, and licensure requirements.
Required Qualifications
- Bachelor's degree in human services or related field
- Minimum of three (3) years of experience working with a specific client group
- Experience working with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, and/or mental illness
- CPR
Desired Qualifications
- Bachelor's degree in human services or related field preferred, with a minimum of three (3) years of experience working with a specific client group
- In lieu of degree, five (5) years of directly related experience required
- Additional training, education, or licensure
