Case Manager III - Adolescent Unit

Job Summary
Assure patients meet care criteria from admission through discharge and initiate authorizations with network providers. Maintain clinical knowledge by reviewing documentation, participating in staffing, attending care conferences, and collaborating with the treatment team. Perform concurrent reviews with providers to monitor medical records and promote compliance for external retrospective review. Liaise with physicians and department members to ensure treatment documentation is complete. Lead information flow between levels of care by coordinating with admissions, clinical, and financial departments to facilitate smooth discharge transitions. Coordinate resources with community services and agencies while discussing continued stay appropriateness with the multidisciplinary team, third-party case managers, and physicians.
Required Qualifications
- Bachelor's degree in Social Work, Counseling or related human service field
- Licensed health care professional with mental health experience (MSW; LCSW; RN; CNS; ARNP; Registered Mental Health Professional (MHP); Counselor (MA; MFT))
- Three or more years' experience working in a behavioral health setting
- Licensure or certification for counseling, as may be required by the state in which the facility operates
- CPR and de-escalation/restraint certification
- First aid
