Job Summary
Conduct thorough client assessments, develop individualized service plans, and coordinate services with the VA, landlords, and treatment providers to support veterans overcoming housing instability and trauma. Provide regular one-on-one meetings and home visits at project-based or scattered-site units to ensure client health and welfare, while transporting individuals to appointments, interviews, and court appearances. Facilitate life skills and therapeutic groups tailored to specialized populations, assist with crisis intervention and after-hours support, and document all services and critical incident reports using electronic client record systems.
Required Qualifications
- Bachelor's degree in social work, psychology, counseling, or related field
- At least two years of work experience in case management or other roles in the social services arena
- Ability to pass background checks if assigned to programs involving incarcerated or recently released clients
- Strong communication, documentation, and organizational skills
- Proficiency in Microsoft Office and electronic records systems
- Valid and current driver's license with clean driving record to meet company insurance requirements
- Ability to complete and pass a driver training course
Desired Qualifications
- Experience working with homeless, veteran or justice-involved populations
- Certification in Substance Abuse Counseling (e.g., CCAPP, CAADE)
- Experience facilitating groups or life skills classes
