Job Summary

Coordinate care plans for members by working with interdisciplinary teams, PCPs, and specialty providers to ensure consistent communication and ongoing treatment goals. Complete comprehensive assessments including CANS-NY and HARP to develop person-centered plans, while actively participating in discharge planning and care transitions following ER visits or inpatient stays. Connect members to community resources addressing social determinants of health, transportation, and chronic diagnoses, and maintain timely documentation to meet DOH regulations. Report any suspected abuse as a mandated reporter and provide monthly care coordination as needed. Work within a fast-paced environment requiring critical thinking, decisive judgment, and minimal supervision.

Required Qualifications

  • Master's degree in one (1) of the qualifying fields
  • Bachelor's degree in one of the qualifying fields
  • Credentialed alcoholism and Substance Abuse Counselor (CASAC)
  • Bachelor's degree or higher in any field
  • Valid NYS Drivers License
  • Proficient with various Microsoft Office products (Word and Excel)
  • Must be able to speak, read, write and follow instructions in English

  • two (2) years of experience as a Health Home Care Manager serving the SMI or SED population
  • three (3) years of experience
  • two (2) years of experience as a Health Home Care Manager serving the SMI or SED population
  • Experience providing direct services to people with serious mental illness, developmental disabilities, alcoholism or substance abuse, and/or children with SED
  • Linking individuals with SMI, children with SED, developmental disabilities, alcoholism, or substance abuse to a broad range of services essential to successful living in a community setting (medical, psychiatric, social, educational, legal, housing and financial services)
  • Ability to organize and establish day-to-day priorities while using critical thinking skills in all aspects of the job
  • Must be able to multitask while remaining professional, focused, composed and positive
  • Excellent customer service skills

  • Must be able to establish an appropriate and effective rapport with others
