Job Summary
Deliver peer support services and case management to participants, dedicating at least 50% of scheduled time to direct client services while assessing needs and implementing individualized service plans. Oversee case management activities to ensure service plans are implemented and goals are achieved, delegating tasks to Case Manager I staff for continuity. Coordinate internal and external care with clinicals and medical providers, support participants in accessing housing and employment, and facilitate classes and support groups. Provide peer-based encouragement using trauma-informed motivational interviewing, assist with transportation, and administer observed urinalysis screenings. Respond to participant crises with de-escalation strategies and activate emergency support as needed. Complete documentation of services, progress notes, and client interactions within 24 hours. This full-time role serves male identifying participants in a behavioral health setting, requiring a valid driver's license and ability to travel to field locations.
Required Qualifications
- Self-identifies as a peer in accordance with OAR requirements, with lived experience in mental health, addiction, gambling, or related challenges relevant to the population served
- Minimum 1 year of continuous sobriety, required and maintained, for those identifying with addiction-related lived experience
- State ORCHARDS BCU background check clearance
- Minimum 1 year of case management experience
- Valid driver's license, personal vehicle with active registration and liability insurance and acceptable driving record in accordance with Bridge's to Change's auto liability policy requirements
- Ability to conduct directly observed urinalysis in a manner that upholds participant safety, dignity, privacy, and trauma-informed care; due to this essential function, identifying as male is required as a Bona Fide Occupational Qualification
- Computer literate; functional ability with Microsoft programs and electronic health record systems
- Demonstrated ability to establish and maintain clear professional boundaries while holding participants and staff accountable with compassion and consistency
- Strong communication, trauma-informed engagement, and conflict resolution skills, with the ability to navigate emotionally charged or complex situations
- Experience with crisis prevention, de-escalation, and intervention strategies in behavioral health or related settings
- Ability to remain calm, regulated, and exercise sound judgment in high-stress or crisis situations
- Ability to manage multiple priorities in a fast-paced, dynamic environment while maintaining attention to detail and participant safety
- Ability to maintain high professional standards in interactions with participants, staff, and community partners
- Demonstrated success working effectively in diverse, collaborative team environments, with strong skills in promoting inclusion and cultural responsiveness
- Experience collaborating with community partners to identify service or system gaps and build partnerships that improve participant outcomes
- Ability to lift, push, pull and move files, supplies and equipment in excess of 50 pounds
- Some or all of the following: Peer Wellness Specialist (PWS) and Qualified Mental Health Associate (QMHA) certifications preferred; at minimum, one credential required with eligibility to obtain the other within 90 days
