Case Manager II

Case Manager II responsible for conducting assessments, writing case plans and providing case management services for clients in schools, hospitals, home visits, and other locations. Responsibilities include client screening and assessment, receiving referrals, developing case plans per program guidelines, coordinating services with paraprofessionals and supervisors, attending staff meetings, providing referral information to families when needed, maintaining client confidentiality, offering curriculum and education to clients/parents, and reporting suspected harm to supervisors and Child Protection Services. Must be able to work independently/remotely, complete required paperwork, and support reaccreditation processes. Requires a Bachelor’s degree in Social Services or related field and driving access; two years of related experience preferred. Salary range: $42,000–$47,000 per year.
