Job Summary
Lead CalAIM Enhanced Care Management (ECM) as a primary point of contact for members with complex medical, behavioral health, and social needs. Conduct comprehensive biopsychosocial assessments, risk stratification, and Care Plan development in alignment with DHCS standards. Lead interdisciplinary care team meetings and coordinate complex transitions from hospitals, SNFs, jails, and residential treatment facilities. Authorize and monitor Community Supports utilization while ensuring documentation meets MCP audit standards. Provide informal mentoring to Case Manager I staff and participate in quality improvement and case review activities. Manage a caseload of approximately 30 to 50 moderate-to-high acuity members, including those experiencing homelessness, justice-involved adults, and individuals with high utilization patterns.
Required Qualifications
- Master of Social Work (MSW) preferred, OR Bachelor's degree in Social Work, Psychology, Nursing, Public Health, or related field
- three (3) or more years of progressive case management experience in Medi-Cal managed care, ECM, Community Supports, Health Homes, Whole Person Care, behavioral health, hospital case management, or community-based care coordination
- Valid California driver's license

Desired Qualifications
- Prior experience as a Lead Care Manager under CalAIM ECM
- Bilingual (English/Spanish)
- ASW, AMFT, or APCC registration
