Job Summary
The Case Manager II position involves providing comprehensive, strength-based case management services to homeless and marginally housed transitional aged youth (ages 18-24). The role requires conducting assessments and developing individualized service plans, linking clients to community resources, and maintaining strong client relationships. Candidates should demonstrate skills in crisis prevention, intervention, and life skills training, along with the ability to document progress and comply with confidentiality laws. A client-centered approach and commitment to diversity, equity, and inclusion are essential, along with some experience in social services.
Required Qualifications
- Some social services case management experience
- Valid driver's license
- Willingness to work holidays occasionally
Desired Qualifications
- Experience working in drop-in services, shelter, transitional and/or permanent supportive housing programs
- Fluency in Spanish and English
