Job Summary
Housing Navigation Center Case Manager II conducts intake and assessments for homeless clients, develops individualized service plans, identifies needed services, coordinates referrals to housing, employment, substance use, nutritional assistance, and other programs, maintains detailed case notes, follows up with agencies to ensure services are initiated and continued, and works with diverse clients in an office and agency-wide setting with after-hours/weekend availability as needed.
Required Qualifications
- Bachelor’s degree in social service field from an accredited college or university preferred, or equivalent combination of education and experience
- Two years of full time professional level casework experience in a social service agency preferred
- Prior experience with homeless population required
- Knowledge and adherence to Social Work or Counselor Code of Ethics
