Job Summary
Provide comprehensive case management and client advocacy for housing programs; engage and assess clients who are experiencing homelessness, assist with housing applications, coordinate entry and access points, conduct intake assessments, and develop individual service plans. Collaborate with community organizations and government agencies to connect clients with resources, support service planning, and ongoing documentation, reporting, and compliance. Duties include outreach (home visits, street outreach, telephone contact), tenancy guidance, client education on rights and responsibilities, maintaining records and progress reports, and contributing to ongoing program improvement. Bilingual (Spanish) preferred; requires strong interviewing, data collection, and Microsoft Office skills, plus valid driver’s license and ability to work in varied environments and driving responsibilities.
Required Qualifications
- High School Diploma or Equivalent
- 2 years of relevant work experience
- Bachelor’s degree in Social Work or related field preferred
- Valid AZ driver’s license
- Fingerprint clearance or ability to obtain
- CPR and First Aid training within 90 days of employment
