Case Manager II - Housing

Provide comprehensive case management and client advocacy within Housing Programs for individuals and families in need. Responsibilities include receiving referrals and engaging clients within 48 hours via home visits or calls; conducting outreach (phone, home visits, street outreach) and coordinating with Supportive Services partners; assisting with housing applications and navigating the housing process (Coordinated Entry, etc.); conducting client intake assessments for emergency shelter and housing programs; managing caseloads for tenancy/pre-tenancy and outreach; collecting and reviewing collateral information to assess eligibility; delivering case management teaching, encouraging client self-advocacy; maintaining up-to-date records, progress notes, and monthly reports; coordinating with community resources; providing information on workshops and financial education; developing and updating individual service plans with quarterly reassessments; ensuring 24/7 emergency telephone coverage in collaboration with staff; and meeting ongoing training and certification requirements. The role requires strong interviewing and documentation skills, fluency with Microsoft Office, understanding of local laws and cultural dynamics, and the ability to work in diverse environments. Bilingual (Spanish) preferred and a High School Diploma with relevant experience is required; a Bachelor’s degree is preferred.
