Case Manager II - Disabled Services

Job Summary

Conduct assessments for disabled residents and families to determine eligibility for housing and benefit programs. Establish and monitor individual care plans while coordinating medical and human services provided by outside organizations. Recruit and maintain an active case load by recruiting and supervising volunteers for various disabled programs. Serve as a liaison between housing management, residents, families, and external providers such as Adult Protective Services and physicians. Assist participants with applications for SNAP, SSI, and Veterans benefits, and facilitate transportation services for Family Self-Sufficiency participants. Maintain accurate confidential records and provide reports on program outcomes to supervisors.

  • Strong clerical, administrative and organizational skills
  • Thorough knowledge of the theories and techniques of case management
  • Thorough knowledge of disabilities services, benefits and programs
  • Attention to details that relate to an individual needs
  • Extensive working knowledge of social and community services programs
  • Skill in human relations necessary to establish and maintain effective working relationships
  • Ability to exercise sound judgment
  • Exhibit professionalism, courtesy and diplomacy with residents, staff, community service agencies and the general public
  • Excellent verbal and written communication skills
  • Ability to effectively deliver public presentations
  • Strong cognitive, analytical and organizational skills
  • Detail oriented in performing and coordinating work, educational and leisure activities
  • Ability to multi-task, plan, organize and schedule work to meet critical deadlines and performance measures

  • Demonstrated strong proficiency in Microsoft Windows application

  • Evening and weekend activities are involved

  • Sufficient manual dexterity to allow operation of a variety of a computer and other office equipment
  • Ability to observe, identify and recognize details at close range

  • Ability to remain in a stationary position 50% of the time
  • Ability to stoop, kneel, bend, stretch, crawl and climb a ladder
  • Constantly positions self to perform the essential functions of the job duties
  • Ability to move items weighing up to 50 pounds across areas
  • Graduation from a college or university of recognized standing in Gerontology, Social Sciences, Human Services or related field of study

Desired Qualifications

  • Demonstrated two (2) years' experience in the field of aging or elderly housing
