Job Summary
Conduct direct case management for adults with serious mental illness and co-occurring conditions experiencing homelessness or housing instability. Assess client needs, develop individualized service plans, and coordinate referrals with community providers to ensure comprehensive care. Support clients in accessing services and benefits while maintaining accurate documentation and case records. Provide education, crisis management, and follow-up services, including in-home visits and client transport when appropriate. Collaborate with supervisors and internal teams for consultation and case planning.
Required Qualifications
- Valid driver's license
- Proof of current insurance if transporting clients
- Experience working with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, and/or mental illness
Desired Qualifications
- Bachelor's degree in human services or related field
- Minimum of two (2) years of experience working with a specific client group
- In lieu of degree, three (3) years of directly related experience
