Job Summary
CASE MANAGER II responsible for client-centered case management across multiple programs (DFSS Interim Housing, No Place to Stay, Veterans Housing, and Recovery Homes). Develop and implement individualized service plans integrating housing, recovery, employment, health, and justice-related needs; conduct comprehensive intake and biopsychosocial assessments; build therapeutic relationships using trauma-informed, strengths-based practices; coordinate with in-house resources and community partners; ensure continuity of care across programs; document progress and ensure compliance; participate in interdisciplinary reviews and provide coverage across programs as needed; advocate for client access to services and fair treatment; provide crisis intervention and de-escalation as required; support recovery and stability goals through engagement and empowerment.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, or Counseling
- Minimum 1–2 years of experience providing case management, counseling, or clinical services in housing, recovery, or justice-involved settings
- Strong knowledge of trauma-informed care, harm reduction, and evidence-based recovery and re-entry practices
- Excellent communication, collaboration, and problem-solving skills
