Job Summary

Conduct persistent, trauma-informed outreach to adults experiencing homelessness with mental health, medical, or substance use disorders. Build trust and assess individual needs to develop service plans, then prepare housing applications and guide clients through subsidized housing processes. Coordinate access to mental health assessments, psychiatric services, and primary care while collaborating with landlords, hospitals, and community partners. Provide crisis intervention, safety planning, and eviction-prevention assistance, transporting clients to appointments as needed. Document all services accurately in the electronic health record and participate in team meetings and quality-assurance activities. Maintain complete case files and support program compliance with contractual reporting requirements.

Required Qualifications

  • Bachelor's degree in social work, psychology, mental health, or a related field
  • At least one (1) year of experience working with individuals experiencing homelessness
  • At least two (2) years of experience providing direct mental health and ICMS Permanent Supportive Housing services
  • Experience working with individuals living with mental illness, chronic medical conditions, and substance use disorders
  • Knowledge of chronic homelessness, outreach and engagement strategies, intensive case management, and evidence-informed service models
  • Experience with crisis intervention, suicide assessment and prevention, affordable housing, public benefits applications, landlord-tenant rights, and eviction prevention
  • Ability to build trust and maintain professional relationships with individuals experiencing significant barriers to care and housing
  • Ability to manage responsibilities independently while traveling throughout the greater Los Angeles area
  • Ability to communicate and collaborate effectively with clients, landlords, healthcare providers, government agencies, and community partners
  • Ability to remain calm, exercise sound judgment, and respond appropriately during stressful or urgent situations
  • Ability to use computers and electronic health record systems to complete timely and accurate documentation
  • A valid California driver's license
  • Clean driving record
  • Insurability under Alcott's automobile liability policy
  • Successful Live Scan fingerprint clearance
  • Driving eligibility throughout employment
