Job Summary
Conduct case management services, crisis intervention, and referral services for residents while providing timely outcome reports per program guidelines. Draft appropriate care plans based on client assessments, maintain up-to-date case notes and files in the Rommel System, and monitor resident progress. Build rapport with clients, facilitate group discussions, and link individuals with community resources, following up on their progress and closing files appropriately. Provide guidance using scriptural references, act as a role model through personal testimony, and lead or participate in devotions, prayers, or outreach events. Keep supervisors informed of challenges and maintain an up-to-date listing of community contacts.
Required Qualifications
- High School Diploma
- previous on-the-job training in Social Work or related field
- Associate's degree
- CAP training
- 2 years' experience
- 1 year experience in working with the homeless population
- ability to lead and facilitate support groups using biblical scriptures and principles
- ability to incorporate scriptures into the rehabilitation process
- ability to exercise independent judgment within the general framework of approved case management techniques
- ability to recognize symptoms of common mental, physical, co-occurring, alcohol/ substance abuse addiction
- ability to make proper assessment for care plans
- ability to provide case management of client services
- ability to provide crisis intervention and referral services
- knowledge of local community agencies, programs and resources available for clients
- ability to establish and maintain effective working relationships with clients and staff of diverse cultural and linguistic backgrounds
- ability to react quickly and calmly in an emergency
- ability to be organized
- computer literate
- ability to sit for lengthy periods of time
- ability to see, hear and speak
- ability to lift and/or carry up to 25 pounds
- ability to move around as needed to perform essential job duties
