Job Summary
As Case Manager I, provide trauma-informed, person-centered case management to adults experiencing homelessness within the Singles Team at Father Joe’s Villages. Conduct intake and holistic assessments, develop and monitor individualized housing-stability plans, coordinate referrals, and advocate for access to housing, benefits, employment supports, and community services. Support clients in accessing Social Security, disability programs, public assistance, and other income supports; encourage engagement in medical and behavioral health services; maintain ongoing engagement, documentation in HMIS, and participate in multidisciplinary case conferencing. Requires collaboration with a multidisciplinary team, strong organizational skills, and ability to manage crises and navigate complex systems to move clients toward permanent housing. Regular travel between worksites is required. Full-time role, typically 40 hours/week, with a starting pay range of $22.00–$25.30 per hour; comprehensive benefits package including medical, dental, vision, retirement, PTO, holidays, and professional development opportunities.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, Sociology, Human Development, or related field, or equivalent experience
- Minimum one (1) year of experience working with underserved or vulnerable populations in a social services or related setting
- Experience working in shelter, housing programs, or human services settings strongly preferred
- Regular travel between worksites and client locations. A valid driver’s license and ability to meet organizational driving standards are required.
