Job Summary
Provide high-quality case management to clients experiencing homelessness, serving as the central point of contact for navigating the transition to tenant living within the community. Collaborate with housing program staff to evaluate eligibility for subsidized housing, prepare and recertify housing vouchers, and conduct regular on-site/home visits. Conduct monthly case management, coordinate referrals to health, mental health, education, and vocational services, and develop individualized service plans addressing housing stability and self-sufficiency. Maintain timely case notes in HMIS and CHAMP within 48 hours; build relationships with property management and community partners; require a Bachelor’s degree in social work or related field and 1-2 years of experience; bilingual Spanish preferred.
Required Qualifications
- Bachelor’s degree in social work, human services, or a related field
- One to two years of experience providing services to highly vulnerable populations
- Ability to work flexible hours including early morning or evenings
- Proficiency in MS Office Suite and internet browsers
- Travel to meet with clients as needed
