Job Summary
Provide services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives under the direction of a case management supervisor. Obtain in-depth social histories and develop agency resources to meet consumer needs. Serve as the Quality Improvement and Development Program (QIDP) coordinator, providing monthly monitoring of the program while ensuring compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. This role involves extensive travel and requires a valid driver's license and car insurance. Candidates must complete the Case Manager Academy Training Program and hold either a bachelor's degree in a human services field with relevant experience or a valid Oklahoma nursing license.
Required Qualifications
- Bachelor's degree in a human services field
- Valid driver's license
- Required car insurance
- One (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field
- Valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing
- Two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities
